SUMMARY: help needed: map heap error

I recently wrote:

The solution for this problem has already appeared on sun-managers,
and is in the Release Notes for 4.1.1. (So who reads the Release Notes?
I don't even know where mine are!) I decided to summarize anyway since
I had not seen the original posting. Solutions proposed:

(1) Run /cdrom/OpenWindows/sun4/lib/OSpatches from the 4.1.1 CD.
(2) Run $OPENWINHOME/lib/OSpatches/install_4.1.1patches.
(3) Run $OPENWINHOME/bin/install_openwin.
(4) Goto 4.1.2.
(5) Check the permissions and major/minor dev numbers on /dev/zero.
(6) Make sure LD_LIBRARY_PATH includes only $OPENWINHOME/lib.
(7) Apply jumbo patch 100257-04 (applicable to
    4.1.1, 4.1.2).
(8) The machine mounted the wrong kvm.
(9) Not enough swap space.

Solution (2) worked for us. Many thanks to the helpful men, women,
